ISPs are arranged with each person in order to set meaningful, personal and vocational goals. These goals form the basis of all service provision and support each person to use a range of services to achieve their aspirations.
Your chosen support team (family, friends and staff) can assist each individual to set and achieve goals.
Wherever possible choice is provided and necessary decision-making skills are taught to develop confidence, allow for self-determination, and support personal growth and development.
This has never been more important as now with the introduction of the NDIS (National Disability Insurance Scheme) set to be fully implemented by July 2017. The aim of the NDIS is to provide freedom of choice and access to the services YOU choose. LFE Community Supports will work with you to tailor an individualised program that links you with the supports and activities of your choice.

The first step is to listen to YOU and explore what opportunities & supports you would like to access.
We will take into consideration your Aspirations & interests, take the time to understand your needs and explore the possible supports available based on your individual requirements.
Once we have gained an understanding of your direction, we will collaborate to design an individual plan that best suits your requirements so you can arrive at your chosen destination.
It is of high importance to LFE Community Supports to ensure of and promote YOUR freedom of choice.
Now that we have a plan we will work with you, your family & carers and assist you to link up with the supports and activities that you have chosen.
