LFE’s Community Connections offers a diverse team of skilled support workers, with a passion for supporting and assisting individuals to work toward achieving their NDIS goals.
How can the Community Connections Team support you?
Community Connections offers comprehensive one on one support and group based activities. We will support you to build your capacity and independence in all aspects of your daily life, including in-home and social supports.
We can support you with a range of activities and supports, including:
Self-Care Activities:
- Personal Care (Including; showering, toileting and dressing)
- Assistance with medication management and support to make medical appointments as well as support to follow medical advice received
- Developing meal plans and shopping lists as well as learning to prepare healthy meals
- Support to learn domestic skills
Social and Community Participation:
- Assisting you to do your grocery shopping and run general errands in the community including accessing; banks, pharmacies, local shops etc.
- Establishing new social networks
- Attending local venues to build your social skills
- Attending both local and out of town events
- Support you to visit family
- Explore a range of transport options
Decision making, daily planning and budgeting:
- Developing a budget that works for you
- Increasing your skill to pay bills on time
- Support you to link with Centrelink and other government agencies
- Assist you with understanding correspondence, such as letters and emails
- Learning how to carry out domestic duties, including; planning healthy meal options, keeping your home clean, washing and gardening
- Developing your skill to manage daily routines
- Assisting you to identify Lifestyle choices to improve your health and wellbeing
- Investigate transport options
Group Based Social Activities
We have a new and exciting group activity program available now.
We encourage all participants to share their interests with us, in order to plan future activities of interest.
Groups activities will cover a range of topics including:
- Life skills development
- Internet safety
- Menu planning
- Relaxation strategies
- Learning how to take care of yourself
- Building independence
- Developing social connections

Facilities
We are very fortunate to have unlimited access to our 100 Main Street Recreation Centre and Day activity center at 8 Mackenzie St.
Some of the exciting activities you can participate in at Main Street (or Mackenzie St), include:
- Craft, painting and gaming
- IT supports
- Musical instruments
- Table tennis
- Air Hockey
- Snooker
- Room to exercise with some equipment available
- Kitchen facilities
- Access to bicycles and a trike
Short Term Accommodation (previously called respite)
We are able to support you to access Short Term Accommodation if funded in your plan.

Currently living in a residential facility?
Our support staff visit many aged care facilities within the Hilltops Region.
Our staff have required vaccinations, and we offer flexible supports in line with current government guidelines.
Staff can deliver supports within the facility or assist you to access your local community.
Needing to change supports or maximise your NDIS funding?
We will work with you, to ensure you receive the right support in the right areas.
When it’s time for your NDIS review, our team will meet with you and prepare a report, ready for your review meeting.
We can also support you during your review meeting, if appropriate.
Who can we support?
We can support anyone with a NDIS plan, aged 0-65 years.
