Toward Recovery Department requires a suitably experienced/qualified person to provide direct and personalised support to people with mental ill-health. PHaMS (Personal Helpers and Mentors) enables people to develop skills, confidence and connection with their community, under their NDIS individually funded plan.

Essential Criteria:

Support Coordination

In the role of Support Coordinator, you will assist National Disability Insurance Scheme (NDIS) participants to coordinate and implement a range of support services, in line with their NDIS plan.

You will also be required to establish strong rapport and working relationships with a range of stakeholders including People with Disability and their family members, NDIS Local Area Coordinators (LAC'S), NDIS Planners and internal and external service providers.

ESSENTIAL CRITERIA:

  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
  • Strong written and computer skills for the creation of service agreements, reports, correspondence and other documents
  • Understanding of the National Disability Insurance Scheme
  • Experience in writing clear, concise and informative reports
  • Demonstrated knowledge of Client Directed Care principles and frameworks
  • Goal orientated
  • High level of confidentiality, self-motivation and ethics

Social & Community Support Worker

LFE Community Supports is seeking honest, hardworking individuals who are passionate about being a mentor/role model to adults and children with challenged abilities who can assist them to work toward their goals as outlined in their NDIS Plans.

 The aim is to support our participants to pursue their passions and achieve their goals while increasing daily living skills, building confidence in social interaction and encourage community connections.
